Patan Hospital is a mission/government hospital located in Kathmandu, Nepal. Each year we see over 300,000 outpatients and do 16,000 admissions, many of these patients coming from the far corners of Nepal. As well as being one of Nepal’s largest hospitals, Patan Hospital is known as a charity hospital where no patient is turned away for any reason. Patan Hospital has a staff of 65 doctors. The hospital is involved in training on a number of different levels. At present there are 30 postgraduate residents and interns working in Patan Hospital. The hospital’s doctor training program includes both active conference and seminar schedule and a fine supervised experience in the ward and clinic. The hospital puts a high emphasis on training staff for outlying hospitals, such as its programs in MDGP anesthesia assistant, nurse midwifery, and ultrasound. Patan Hospital is involved in negotiations with KUMS regarding Patan Hospital providing a site for clinical training of medical students.

Significant numbers of our Internal Medicine and Pediatric patients have cardiac conditions. There are still many patients here with undiagnosed congenital heart disease and advanced rheumatic heart disease. Ischemic heart disease is increasing each year. Our Internal Medicine team refers an average of one patient per day for echocardiography. These patients have to go outside to have their echo done in various clinics around the city. Although the echos are done by supposed cardiologists, our Medicine and Pediatrics teams have found the reliability of the reports to be low. At the same time, cardiac surgery has been developing rapidly and there are more options for moderately priced operations. This increases the need for making accurate diagnosis on certain cardiac conditions.

Given the increasing demand for reliable echocardiography, the number of patients being referred out to clinics in Kathmandu, and the lack of uniform quality of the reports - Patan Hospital is developing its own capability for echocardiography. During the last two years church groups in Upstate New York have raised $75,000 for the purchase of a new echo/ultrasound machine. This refurbished state of the art machine should arrive in Nepal in late 2003 and be ready for use in Patan Hospital. Although the trend in Nepal is to have cardiologists doing all echocardiography, my experience in the United States is that this examination can be performed also by trained internists. We in Patan Hospital would like to send one of our staff for intensive training in echocardiography. We believe that if the staff was well selected and the training in the United States well implemented, that this could result in provision of an excellent service for our patients.

We perceive that the ANMF strength lies in its network. Rather than ask for funding from ANMF, we would prefer to seek help in the area in which it is strongest. In that regard, we would be looking for help according to the following plan:
  1. ANMF would identify a cardiologist/team in the United States who were qualified and motivated to take on this 3-6 month project.
  2. Patan Hospital and ANMF would seek the advice of the US cardiology team with regard to the best implementation of this training. This might involve some reading or experience in Nepal before the Patan Hospital staff member came to the United States.
  3. Based upon US recommendations, the Patan Hospital doctor would travel to the United States for 3-6 months of intensive training in echocardiography. ANMF would pay for travel and room and board. Its not anticipated that there will be a high cost of the training, but that depends upon finding the right partner in the United States.
